Thank you so much for your interest in enrolling your child into Resonant Vibe's R.I.S.E. Youth Mentoring program!

We appreciate your desire for your child to have a mentor.  R.I.S.E. stands for Resilience Inspiration Support Empowerment. Our goal is to provide youth with a caring adult who can be a part of their support system and help them to RISE!  

All mentors are volunteers, and because we rely heavily on volunteers, the length of time before your child will be matched can vary. We also match based on mutual interests, age, and locations. Unfortunately, this means some youth wait longer than others, but it gives each child the best chance to build a long lasting friendship with their mentor.

As soon as you submit this application, your child will be placed on our waiting list. While you are waiting for your child to be matched, keep an eye on your email and text messages because we host monthly events called Vibe Hangs for Families. These are connection events hosted by Resonant Vibe that will feature workshops, speakers, and fun activities. More information about Vibe Hangs.

Please keep us updated with any changes to your address, phone number, and email address. This is very important because we will use this information to reach out to you to schedule an intake interview. If we cannot reach you, your child's application will have to be closed.

When it is time to start the pre-enrollment process for your child, we will first attempt to reach you by text and email. If you prefer another form of communication, please indicate it in this application.

At Resonant Vibe, we care deeply about the safety, health and overall well-being of the children we serve and their families. The following questions will help us to identify where your family may want some support and how we can help. These questions also help us identify where your child's mentor may also need support while they are matched with your child.

The following questions are designed to help us make the best, most successful match
for you, your child, and a potential volunteer. We do this by exploring your preferences
in a match, as well as the preferences of your child and prospective volunteers to
ensure that everyone feels comfortable. Big Brothers Big Sisters is an inclusive
organization. We welcome and enroll Bigs, Littles and family members of all races,
religions, marital status, genders and sexual orientations. We are committed to making
sure our programs are inclusive of all youth, families, volunteers, and staff. The goal of
the matching process is to provide high quality, long-lasting mentoring relationships
which have the maximum benefits for a child while providing a fulfilling experience for
the volunteers.




For safety reasons, we are required to obtain verbal consent for your child to participate in programming from the parent who shares custody of your child.

Video/Photography/Social Media Consent

Resonant Vibe allows mentors, youth, and families to use digital communication only when it supports healthy mentoring and respects family preferences. Parents must decide whether digital communication or social media involvement is allowed.


No photos, videos, names, tags, or identifying information about a youth may be shared without written parent/guardian consent. If consent is not given, all digital references must stay general and non‑identifying.


When digital interaction is allowed, mentors must use privacy settings, keep content youth‑appropriate, protect family information, and report any safety concerns to Resonant Vibe.


Resonant Vibe may review concerns about inappropriate online activity. Violations of the policy may lead to guidance, suspension, or ending the mentoring relationship, with youth safety always the priority.

  • I hereby grant Resonant Vibe the irrevocable right and permission to use photographs and/or video recordings of my youth in publications, promotional materials, educational materials, derivative works, or similar purposes without compensation to me.
  • I understand and agree that these photographs or videos may be placed on the Internet.
  • My youth may be identified by first name only in printed, online, or broadcast materials connected to these images.
  • I waive the right to inspect or approve the final product.
  • All photographs, videos, audio recordings, and digital files remain the sole property of Resonant Vibe.
  • I release and discharge Resonant Vibe and its representatives from any claims or liabilities related to the use of these materials, including claims for invasion of privacy, appropriation of likeness, or defamation.
  • I understand that I may revoke this consent in writing at any time, and until revoked, this release remains valid.




  • I understand that the mentor may share photographs and/or videos of my youth on their personal social media or digital platforms.

  • I understand that my youth may be identified by first name only.
  • I waive the right to review or approve the final posts or materials.
  • I understand that the mentor is responsible for following Resonant Vibe’s privacy and youth‑safety expectations.
  • I understand that I may revoke this permission in writing at any time, and until revoked, this consent remains valid.


This section governs use of photos, videos, and identifying information by Resonant Vibe as an organization, separate from mentor use.

Consent for Surveys and Training
I give permission to have my child and myself complete questionnaires and training
containing topics and questions about peer relationships, feelings about school, grades,
educational expectations, parental relationships and attitudes toward school. 
Consent to Release Information

Participation Agreement

PERMISSION FORM & RELEASE FOR PARTICIPATION IN RESONANT VIBE

 

By confirming below, I, as the legal guardian, give full permission:

  1. For my child to participate in the Resonant Vibe Program.
  2. For the volunteer matched with my child, who has been screened and approved by Resonant Vibe, to transport my child to events and match activities.
  3. For the school to provide social and academic information about my child to Resonant Vibe (e.g. report cards, behavior reports);
  4. To have my child participate in an in-take interview conducted by Resonant Vibe staff and complete questionnaires throughout his/her time in the program containing questions about school, home life, and personal interests.
  5. To have my child talk with a Resonant Vibe staff person about personal safety.
  6. For Resonant Vibe staff to provide contact information for me and my child to the volunteer.
  7. For Resonant Vibe staff to meet with my child at their school and/or afterschool program center to conduct an interview and/or match support.
  8. I give permission for my child’s Mentor to meet with them at their school.

 

I understand that the program is not obligated to match my child with a volunteer and that as part of the enrollment process, I will be asked to provide additional information through an in-person interview. 


I understand that the information I provide in the enrollment process will be kept confidential unless disclosure is required by law and with exceptions noted.  


I understand that incidents of child abuse or neglect, past or present, must be reported to proper authorities. 


I understand that certain relevant information about my child will be discussed with the volunteer who is a prospective match (i.e., demographic information, information relevant to volunteer preferences, and information relevant to child-safety and well-being).

 

I certify that all of the information on this form is true and correct and that all income is reported. I understand this information is being given for the receipt of federal funds, that the information on this application may be verified, and that deliberate misrepresentation of the information may subject me to prosecution under applicable state and federal laws. I understand this information will not affect my family's qualification for the program.

 

I do hereby release the organization and its employees, agents, members, volunteers, and all other persons on its behalf from any and all liability for any damage or injury which such child might sustain while participating in said program and activities, including but not limited to any liability to any right of action that may occur to such child directly, or to me as their guardian. I understand that this information may be shared with the school or with partnership agencies when applicable.

 

If my child is matched with a Mentor, I agree to support my child’s match by reviewing the program and safety information given to me by Resonant Vibe, communicating with Resonant Vibe staff as outlined in expectations (which includes communication at least once a month), and immediately reporting any concerns I might have to Resonant Vibe staff.
